Outreach Case Management
Case management and outreach are two important services provided to many individuals, particularly people with multiple problems regardless of ability to pay or having insurance. With little to no barrier to entry, any adult experiencing homelessness, food insecurities, or in need of mental health or substance use services may receive support without following strict protocols like attendance policies.
Our Outreach case management team make regular outreach or on-site visits to individuals living in encampments or other locations throughout the various community based sites that we partner with.
